In which case, as a pensioner, you are responsible the state pays health insurance premiums from the statutory assessment base for state insured persons (from 1 July 2020 the insurance premium is CZK 1,567.- from the assessment base of CZK 11,607.-).
But besides that. you must also pay insurance premiums on your earned income like the others. However, you are not obliged to pay at least the minimum health insurance premium, but only the amount actually calculated.
If you're self-employed, you pay a monthly premium. If you are an employee, you pay your premiums through your employer. He calculates insurance premiums on the actual assessment base, i.e. your actual earnings, and does not make any additions to the minimum assessment base (which is always set at the current minimum wage - this year CZK 14,600).
You must notify your health insurer if you want to be classified as a person for whom the state pays the premiums (i.e. if you want to be classified as a state insured person). If you are an employee, your employer (if he/she knows this) will make the notification. It is certainly advisable to check with your employer or directly with the health insurance company that the employer has made the notification.
If you notify the insurance company yourself, you must provide a document showing when you were granted an old-age pension, i.e. the "Decision on granting the old-age pension".

Setting up a pensioner is done on the employee tab - tab Personal data / Employee, area Retirement. You can save his/her classification as a state insured in the Health insurance - field "ZP relief":
Notification to the relevant health insurer is made using the "Employer's Bulk Notification" form. The award of retirement pension is shown under code "D". You notify the pensioner's entry into employment on two lines - one with code P and the other with code D.
To print or electronically send a notification to the health insurance company, use the menu in the program Main menu - Press reports - Health insurance companies - Employer's mass notification.
